Activity Waiver and release of liability

Please read and fill out the required Activity Waiver and Release of Liability before you book your tee time. You will be required to have a Liability Release on file before visiting Blade Bros. Golf Club. Please ensure all your guests have read and signed a liability release to prevent any delay to the start of your round. Your Liability Release will only need to be renewed once per calendar year.
